Sports Physicals
FREE Sports Physicals are back for 2024 at Tennessee Orthopedic Alliance in Columbia. Boys Physicals are April 29th and Girls Physicals are May 6th. ALL local athletes attending Maury County Public Schools, Zion Christian Academy, Agathos Classical School, Columbia Academy, and Columbia State Community College are welcome to attend.
All Cross Country Elementary and Middle School students need to be in line by 5:30 and all other athletes in line by 6:00 each night. Students, please wear t-shirts, shorts, and athletic shoes. Coaches are encouraged to attend for crowd control.
Student-athletes must bring the following signed paperwork:
- Examination Form (Link)
- Students are to fill out their name and date of birth at the top of the form.
- The rest of the form will be filled out by the physician on-site during the physical.
- History Form (Link)
- Parents/guardians and students MUST fill out the form in its entirety prior to the physicals.
- Parents/guardians MUST sign the bottom of the form. Without a signature, the student will not receive a physical.
April 29th (Boys) and May 6th (Girls) are the only dates TOA Columbia will provide free physicals. If athletes are unable to attend, they will need to see their primary care physician.
This is a long-standing tradition in our community (since 1978 to be exact.) Meeting new athletes, parents, and coaches as well as visiting with the friends we’ve made along the way is one of our favorite community activities.
